If told to report to the Assessment and Registration Center, please bring the following items for each child.

• Blue copy of the Home Language Survey.
  
• Proof of birth date.
  
• Immunization record.
  
• Proof of physical examination during the last year (first grade only).
  
• Home address and telephone number.
  
• Address and telephone number of parents or guardian's employer.
  
• Name and telelphone number of two emergency contacts
  
• Residency Permit: If the student does not live with the parent or legal guardian, please go to the Garden Grove Unified School District, Office of Child Welfare and Attendance. This office is located on the third floor at 10331 Stanford Ave. You must complete a Residency Permit.
  
• Additional Information: Please bring social security and/or immigrant or refugee numbers if you have them.

Please be advised that the testing procedures could take up to 2 hours for grades K-6 and 3 hours for grades 7-12. We look forward to assisting you in registering your child.
